Qualifications

• Graduate of accredited University program in Physical Therapy.
• Indiana Licensure or eligible for state licensure within 180 days.
• CPR certification current, BLS Course C.

Responsibilities

• Reviews and evaluates physician’s referral to determine physical therapy treatment, goals, plans and patient education.
• Assess progress and goals, modify treatment plan as appropriate.
• Review patient medication list, allergies and medical and surgical history.
