Job summary

Brooks Rehabilitation in Jacksonville is seeking a skilled Physical Therapist to join our Outpatient team. This float position works across clinics in the Jacksonville region, Monday through Friday with PRN coverage, delivering patient-centered care and collaborating with a multidisciplinary team.

You will conduct new patient evaluations, develop customized treatment plans, and progress therapy across multiple diagnoses while promoting evidence-based practice and exceptional patient experiences.

Qualifications

• Current PT license in Florida state.
• CPR/BLS certification required.
• Knowledge of federal and state professional requirements.

Responsibilities

• Complete the new patient evaluation and establish customized treatment plans and goals.
• Deliver high-quality patient care for multiple rehabilitation diagnoses.
• Provide a positive patient experience through patient engagement and progressive treatment.
• Effectively communicate verbally and in writing.
• Promote evidence-based patient care.
