Job summary

Northwell Health seeks a Physical Therapist for Danbury Outpatient to deliver evaluation, treatment, and discharge planning. This full-time role emphasizes patient education, team coordination, and adherence to department guidelines within a comprehensive outpatient setting.

Qualifications

• Minimum Experience: one year.
• Connecticut State Physical Therapy License is required.
• FIM certification in Physical Therapy rehab settings. CPR Certification. Bachelor’s degree and minimum one year of experience.

Responsibilities

• Performs therapy evaluations, treatment planning and implementation utilizing department clinical guidelines & protocols.
• Provides patient/ family education and training.
• Documentation is completed based on department guidelines.
• Initiates discharge planning involving patient, family and team members as appropriate.
• Attends 20 hours of continuing education per year.
• Provides full-time clinical experience to students.
• Acts as mentor to lower level staff.
• Performs other duties as assigned.
