Physical Therapist - OP Houghton Lake, Up to $20,000 Bonus

Primary Location: SITE - Outpatient Prudenville - Prudenville

Department Name: Houghton Lake/Prudenville Outpatient

Employment Type: Full time

Shift: Days

Weekly Scheduled Hours: 40

Hours of Work: 8:00am - 5:00pm

Compensation: Up to $20,000 Sign on Bonus, Starting at $36.54 (eligible for increase based on experience), Annual Merit Increases, Medical, Dental, Vision Benefits

Age Specific Responsibility: Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served in assigned department.

Summary: Assumes full responsibility for evaluating patient’s need for physical therapy. Plans and executes individual treatment programs in accordance with the State of Michigan licensure and PT scope of practice.

Essential Job Responsibilities:

  • Provides patient care, treatment, and services within the scope of their license, certification or registration and as required by law and regulation.
  • Evaluates patient condition and establishes treatment goals in collaboration with patient and family.
  • Re-evaluates patient's status and effects of treatment. Revises treatment plan as necessary.
  • Evaluates patient's need for medical equipment and orders the appropriate equipment as necessary.
  • Participates in planning for patient discharge.
  • Documents patient's status and progress according to established standards.
  • Communicates with others to integrate physical therapy treatment with all aspects of patient care.
  • Attends various meetings and in-services as designated.
  • Meets or exceeds program productivity expectations.
  • Adheres to the Mary Free Bed Standards of Behavior.

Customer Service Responsibilities: Demonstrate excellent customer service and standards of behaviors.

Responsibilities in Quality Improvement: Contribute by identifying problems and seeking solutions.

Essential Job Qualifications:

  • Graduate of Commission on Accreditation in Physical Therapy Education (CAPTE)
  • Licensure is required in every US State and Territory the Therapist is practicing.
  • Possesses and maintains BLS certification.
