Job summary

Baptist Health Medical Group Eastpoint is seeking an Outpatient Physical Therapist to evaluate and treat patients in our Louisville, KY clinic. The PT will implement individualized care plans, using skilled therapy techniques and patient education to promote recovery and functional improvement.

The position requires an active Kentucky PT license or temporary license, and BLS certification within 90 days of hire.

Qualifications

• Current active Kentucky Physical Therapy license (or temporary license)
• BLS certification within 90 days of hire
• Ability to work 40 hours per week with potential early/late shifts
• New graduates encouraged to apply

Responsibilities

• Evaluate patient health needs in outpatient clinic and develop an appropriate plan
• Provide skilled physical therapy procedures and educate patients and families in the plan of care
• Document treatment and progress using electronic records
• Collaborate with physicians and care team to optimize patient outcomes
