Job summary

DayToDay Health is seeking a licensed Physical Therapist to deliver evidence-based outpatient care within our Healthcare & Medical Services department. You will evaluate, diagnose, and treat a diverse patient caseload, designing individualized treatment plans and collaborating with an interdisciplinary team to ensure comprehensive care.

Qualifications

• Active Physical Therapy license in the United States
• Proven ability to develop personalized treatment plans for outpatient clients

Responsibilities

• Evaluate, diagnose, and treat a diverse outpatient caseload
• Design individualized therapy plans tailored to each patient
• Utilize therapeutic exercise, manual therapy, and modalities to restore function
• Collaborate with an interdisciplinary team to ensure comprehensive care
• Educate patients and families about treatment plans and health management
• Document progress in electronic medical records (EMR)
