Job summary

Wake Forest Health Network, LLC is seeking a licensed Physical Therapist in North Carolina for outpatient care. The role requires a BS in Physical Therapy, NC licensure, and CPR certification renewed every two years.

You will perform comprehensive patient evaluations and establish treatment plans, documenting progress and outcomes in a timely manner. The team focuses on quality care and professional development.

Qualifications

• Bachelor of Science degree from an accredited physical therapy school required.
• Licensed to practice Physical Therapy in North Carolina required.
• CPR certification required every two years.

Responsibilities

• Provides patient care assessment and treatment following Standard of Practice.
• Completes patient evaluations, progress notes, discharge notes and education documentation.
• Maintains accurate patient documentation and ICD-9 coding as appropriate.
• Meets department productivity and budgeted expectations.
• Adjusts schedules to meet patient and department needs.
• Participates in process management to improve quality and efficiency.
• Supervises students to develop clinical skills and ensure quality care.
• Communicates professionally with patients, physicians, staff and external customers.
• Adheres to hospital standards and promotes a cooperative work environment.

Education

Bachelor of Science degree from an accredited physical therapy school

Licensed to practice Physical Therapy in the State of North Carolina

CPR certification

APTA membership encouraged
