Hallettsville, TX-based Complete Hometown Physical Therapy, a H2 Health company, seeks a Physical Therapist for outpatient care. You will evaluate patients, develop individualized plans of care, and deliver evidence-based interventions to improve mobility, strength, balance, and function.

Formación

• DPT from a CAPTE-accredited program.
• Texas PT license or eligibility for licensure.
• CPR certification or willingness to obtain.
• New graduates are encouraged to apply.

Responsabilidades

• Perform comprehensive patient evaluations and develop individualized plans of care.
• Deliver evidence-based treatment interventions to improve mobility, strength, balance, function, and overall quality of life.
• Treat orthopedic, post-operative, musculoskeletal, balance, dizziness, and mobility-related conditions.
• Collaborate with patients, families, physicians, and interdisciplinary team members to achieve optimal outcomes.
• Educate patients on injury prevention, wellness, and home exercise programs.
• Maintain accurate and timely documentation of patient progress and treatment outcomes.
• Utilize Scribe AI-powered documentation to streamline documentation and maximize patient care time.
• Utilize the Raintree EMR platform for documentation, scheduling, and patient management.
• Work within a manageable caseload environment, typically targeting ~10–12 patient visits per day.
• Contribute to a positive clinic culture focused on patient-centered care and clinical excellence.
