Job summary

H2 Health is seeking a Physical Therapist for our outpatient clinic in Salem, Virginia. You'll assess patients, develop individualized plans, and provide evidence-based care across orthopedic, post-operative, neurology, vestibular, balance, and mobility needs. New graduates are welcome.

You must hold Virginia PT licensure (or eligible), be CPR-certified, and be willing to travel between clinic locations. Expect a collaborative environment with mentorship and growth opportunities within the H2

Qualifications

• DPT or MSPT/MPT/BPT from CAPTE-accredited program.
• Current Virginia Physical Therapist license or eligibility.
• CPR certification or willingness to obtain.
• Ability and willingness to travel between clinic locations.
• New graduates encouraged to apply.

Responsibilities

• Perform comprehensive patient evaluations and develop plans of care.
• Deliver evidence-based treatment to improve mobility, strength, balance, function.
• Treat orthopedic, post-operative, neurological, vestibular and mobility conditions.
• Collaborate with patients, families, physicians and team members for outcomes.
• Educate patients on injury prevention and home exercise programs.
• Maintain accurate documentation of progress and outcomes.
• Utilize AI-powered and EMR tools to streamline documentation.
• Manage a typical 10–12 patient visits per day in outpatient setting.
• Support a positive clinic culture focused on patient-centered care.
