Job summary

LiveWell Homecare Agency is seeking a full-time licensed physical therapist to provide outpatient services for orthopedic, sports medicine, and neurologic patients in a community-based facility. A flexible schedule offers 4 ten-hour shifts or 5 eight-hour shifts, with late days until 6 pm and weekend rotations.

You will develop personalized treatment plans, educate patients and families, document thoroughly, and collaborate with healthcare teams to ensure safe, high-quality care.

Qualifications

• Bachelor’s Degree in Physical Therapy.
• Licensed as a Physical Therapist.
• Experience is not required; supervision experience is also not necessary.

Responsibilities

• Establishes treatment plans considering discharge needs and functional outcomes.
• Provides patient education and collaborates with healthcare providers, making referrals as needed.
• Delegates interventions based on skill levels, ensuring safe and effective care.
• Completes accurate documentation and conducts assessments, reassessments, and consultations.
