Job summary

Brooks Rehabilitation is seeking a skilled and compassionate Physical Therapist to join our Outpatient team at Fernandina Beach, FL. This part-time role offers 20 hours per week with potential to pick up additional hours at Amelia Island, Yulee or Callahan locations.

You will deliver high-quality patient care, evaluate new patients, and develop individualized treatment plans in a collaborative clinical environment. Qualifications include an active Florida PT license and CPR/BLS certification.

• CPR/BLS certification.
• Knowledge of federal and state professional requirements.

Responsibilities

• Complete the new patient evaluation, establish customized treatment plans and patient goals, and participate in the implementation of patient care programs.
• Deliver high quality patient care for multiple rehabilitation diagnoses.
• Provide a positive patient experience through patient engagement and progressive treatment.
• Effectively communicate both verbally and written.
