Job summary

Northwestern Medicine is seeking a motivated Physical Therapist for outpatient Huntley/Crystal Lake clinics, full-time. You will deliver direct patient care, manage plans of care, and work within a collaborative healthcare team.

The role requires IL licensure and a BLS certification at hire, with potential involvement in teaching and process improvement. The position divides time between Huntley and Crystal Lake locations, with a 40-hour work week, no weekends or major holidays.

Qualifications

• Bachelor’s Degree in Physical Therapy.
• Licensed Physical Therapist in State of Illinois.
• BLS Certification from American Heart Association upon hire.
• Masters or Doctoral Degree in Physical Therapy preferred.

Responsibilities

• Direct patient care and management of the patient plan of care.
• Collaborate with PT/PTA staff and other clinicians to coordinate treatment.
• Maintain EMR documentation including evaluations, progress notes, and discharge summaries.
• Supervise and mentor clinical interns or students as assigned.
• Participate in department committees, processes, and continuous improvement efforts.
