LHM Physical Therapy Institute is seeking a Physical Therapist - Outpatient to join our team on a full-time salaried basis. This role focuses on evaluating and treating patients using diagnostic functional testing and developing individualized therapy programs in an outpatient setting.

Responsibilities include coordinating care with the care team, supervising rehab tasks, documenting care, and adhering to policies while delivering compassionate, patient-centered therapy across our community.

Qualifications

• Graduate from an accredited physical therapy program.
• Holds Pennsylvania Physical Therapist License.
• Team oriented culture and approach.

Responsabilités

• Evaluate patients using diagnostic functional testing in accordance with the Pennsylvania PT Practice Act.
• Plans individuals physical therapy programs for each patient including exercise and functional training in a safe manner.
• Provides direction and supervision for patients with appropriate level of support including active, active assisted, passive exercises, neuromuscular reeducation, and functional transfers and training using appropriate assistive devices and equipment as appropriate.
• Coordinates Physical Therapy intervention and care with other participants on the care team.
• Participates in educational in-services for team members directly and indirectly connected with the therapy department.
• Delegates appropriate rehab tasks to support personnel including physical therapist assistants and rehab assistants as needed
