Job summary

Sequoia Care in Lakeland, FL is seeking licensed Physical Therapists (PTs) to join our outpatient team on a PRN/part-time basis. You will assess, develop, and implement personalized treatment plans to help residents regain movement and reduce pain in senior living and home health settings.

We offer a flexible schedule with competitive hourly rates for PTA $32-40 and PT $47-55, plus a monthly $250 travel stipend.

Qualifications

• Doctorate or Master’s in Physical Therapy from an accredited program.
• Current Physical Therapist license in Florida (required).
• Senior living or similar experience preferred.
• CPR certification as required by facility or state.
• Strong clinical, documentation and communication skills.

Responsibilities

• Perform evaluations and develop individualized treatment plans.
• Deliver skilled physical therapy services to residents in a senior living setting.
• Monitor and document resident progress, adjusting plans as needed.
• Educate residents, caregivers, and staff on mobility, exercise, and prevention strategies.
• Collaborate with interdisciplinary teams to ensure coordinated, high-quality care.
• Ensure compliance with Florida licensure and documentation standards.
