Qualifications

• 1+ year outpatient clinical experience
• Active Physical Therapy license in the state where services will be provided
• Current CPR certification required
• Strong clinical reasoning, communication, and organizational skills
• Ability to develop and implement individualized treatment plans
• Commitment to patient-centered care and positive clinical outcomes
• Ability to work effectively in a collaborative healthcare environment

Responsibilities

• Conduct comprehensive physical therapy evaluations for assigned patients.
• Develop individualized treatment plans based on patient needs, goals, medical history, and physician direction.
• Provide 60-minute evaluation and treatment sessions.
• Implement, modify, and progress treatment plans based on patient response and clinical needs.
• Develop functional treatment programs designed to improve mobility, independence, and quality of life.
• Educate patients and families on treatment plans, therapeutic exercises, home programs, and injury prevention.
• Monitor and document patient progress accurately and efficiently.
• Maintain complete and timely medical records in accordance with applicable regulations and clinic policies.
• Provide care in accordance with valid physician orders and applicable insurance authorizations.
• Collaborate with other healthcare professionals to coordinate patient care.
• Participate in quality improvement initiatives.
• Maintain professional licensure and required continuing education.
• Follow applicable state practice acts, professional standards, and ethical guidelines.
• Promote a safe environment for patients, coworkers, and visitors.
