Job summary

Azza HealthCare Agency is seeking a licensed Physical Therapist to provide evidence-based outpatient physical therapy in a patient-first, collaborative setting. Evaluate and treat musculoskeletal and neurologic conditions, developing individualized plans of care to restore function, independence, and quality of life.

Responsibilities include conducting assessments, creating care plans, delivering evidence-based interventions, maintaining EMR documentation, and coordinating care with physicians

Qualifications

• Licensed Physical Therapist with current license.
• Outpatient rehabilitation experience preferred.
• Strong documentation skills in EMR.
