Job summary

Trinity Health Ann Arbor is seeking a Community Outpatient Pharmacist to provide comprehensive pharmaceutical care for outpatient patients, supervise technicians, and support immunizations and medication therapy management.

You will collaborate with physicians, nurses, and other health professionals to ensure safe, effective, and cost-conscious drug therapy, counsel patients, and participate in teaching and quality improvement activities.

Qualifications

• Broad knowledge of medications, compounding, and dispensing.
• Interpersonal skills necessary to communicate effectively with a wide variety of hospital personnel and diverse patient population.
• Excellent customer service orientation and clear written/oral communication.

Responsibilities

• Interprets and evaluates all prescriber's medication orders to ensure appropriate drug therapy.
• Reviews and screens patient orders for allergies, interactions, contraindications, and dosing.
• Communicates with physicians and patients to optimize therapy; document interventions.
• Performs patient medication histories and counseling as required.
• Consults with physicians on safe, rational, cost-effective medication plans.
• Directs and monitors pharmacy technicians and interns in preparation and distribution of medications.
• Administers vaccinations to patients and maintains records.
• Participates in teaching programs and training for staff and students.
• Maintains compliance with federal, state, and local laws and regulations.
