Qualifications

• Graduate from an accredited Physical Therapy Program.
• Current State of Delaware Physical Therapist license (PT), CPR certification.
• Outgoing and energetic personality.

Responsibilities

• Assess patients to develop and initiate treatment plans based on evaluations.
• Review physician referrals and medical records to determine required therapy.
• Provide direct patient care by established protocols and guidelines.
• Document findings, progress, and instructions for patients and caregivers.
• Oversee care by assistants and rehabilitation technicians.
• Develop written plans of care and comprehensive programs to meet goals.
• Implement therapy treatments and ensure essential services are delivered.
• Communicate with patients, caregivers, physicians, and insurers.
• Complete timely documentation including daily notes and progress reports.
