Job summary

Optimum Performance Therapy in Warsaw, IN is seeking a Licensed Physical Therapist for a full-time, dayshift role in an outpatient orthopedic setting. You will evaluate patients, develop treatment plans, and provide therapies with regular progress reassessment.

You will collaborate with physicians and the team, educate patients, and ensure documentation and safety standards are met. Relocation assistance and a sign-on bonus are available.

Qualifications

• Bachelor’s Degree in Physical Therapy is required.
• 0-2 years of experience in a physical therapy setting required.

Responsibilities

• Conduct thorough PT evaluations and develop individualized treatment plans.
• Administer safe therapy treatments and document interventions.
• Reassess and adjust plans based on patient progress.
• Document evaluations, treatment plans, and billing information clearly.
• Educate patients and families on exercises and post-treatment care.
• Operate therapy equipment safely and maintain a clean environment.
• Collaborate with physicians and interdisciplinary teams for continuity of care.
