Job summary

Qualifications

• Bachelor’s degree or equivalent combination of education and experience.
• RN licensure required; five years recent clinical experience preferred.
• Knowledge of clinical practice guidelines, payer protocols, and use of computer databases.

Responsibilities

• Facilitate patient navigation from pre-admission through post-discharge coordination.
• Monitor and coordinate the plan of care to improve timeliness and throughput.
• Collaborate with physicians, nurses and other health care workers to manage transitions.
• Apply evidence-based criteria to ensure appropriate levels of care.
