Occupational Therapist (OT) - Outpatient Home Health

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for outpatient home health visits at our Montgomery Maryland Senior Living Office in Gaithersburg, MD. This office serves adult and geriatric patients in Senior Living Facilities across Montgomery County.

As a home care Occupational Therapist, you will be a vital part of a multi-disciplinary team providing rehabilitative care and skilled nursing, enabling clients to receive necessary medical care while remaining at home.

Responsibilities:

• Conduct outpatient therapy evaluations to develop care plans for patients in senior living communities or group homes for adults with intellectual disabilities.
• Operate professionally at the top of your license, using current research-based approaches to maximize functional independence.
• Collaborate with therapy partners to transition care from home health to outpatient services, achieving client-centered goals in their living environment.
• Educate clients, families, and caregivers on care plans to promote a team approach to maximizing independence.
• Perform evaluations, treatments, and discharge planning using our outpatient EMR platform to document and deliver personalized care.

Qualifications:

• Current Maryland Occupational Therapist license.
• Prior home care experience preferred but not required.
• Minimum of one year recent experience as an Occupational Therapist.
• Graduation from an accredited occupational therapy program (AMA and AOTA approved).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; EMR experience preferred.
