BAYADA Home Health Care is currently seeking an experienced Occupational Therapist (OT) for a Full-Time outpatient home health opportunity in our Wilmington Senior Living Office, Wilmington, DE. This role involves providing outpatient visits to adult and geriatric patients in homes throughout the Hockessin area.

If you want to grow your outpatient skills in a setting that emphasizes quality over quantity and allows for more personalized patient care, this position is ideal for you.

As an occupational therapist in home care, you will be a key member of a multidisciplinary team delivering rehabilitative and skilled nursing care to help clients remain at home.

Occupational Therapist Responsibilities:

1. Perform outpatient therapy evaluations to develop individualized care plans for patients in senior living or group home settings for adults with intellectual disabilities.
2. Operate at the top of your license using current, research-based approaches to maximize clients' functional independence.
3. Collaborate with therapy partners to transition care from home health to outpatient services, achieving client-centered goals in their living environment.
4. Educate clients, families, and caregivers on care plans to promote a team approach to independence.
5. Conduct evaluations, treatments, and discharge planning using our outpatient EMR platform to document personalized treatment approaches.

Qualifications:

• Current DE Occupational Therapist license.
• Preferred: prior home care experience; not required.
• At least one year of recent OT work experience.
• Graduation from an accredited occupational therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
