BAYADA Home Health Care is currently seeking an experienced Occupational Therapist, OT, for a Full time opportunity performing Outpatient home health visits.

This office services adult and geriatric patients on a per visit basis in homes throughout York County.

As a home care Occupational Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, enabling them to remain at home.

Occupational Therapist, OT Responsibilities:

1. Perform outpatient therapy evaluations to develop a plan of care for patients in senior living communities or group homes for adults with intellectual disabilities.
2. Operate professionally at the top of your licensure using current research-based approaches to maximize functional independence.
3. Collaborate with home health therapy partners to transition care from home health to outpatient services, achieving client-centered goals in their living environment.
4. Educate clients, families, and caregivers on care plans to promote a team approach to maximizing independence.
5. Perform evaluations, treatments, and discharge planning using the outpatient EMR platform to document and deliver individualized care.

Occupational Therapist, OT Qualifications:

• Current PA Occupational Therapist license.
• Prior home care experience preferred but not required.
• Minimum of one year recent work experience as an Occupational Therapist.
• Graduation from an accredited occupational therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
