Qualifikationen

• Licensed VA Occupational Therapist with current credentials.
• Minimum of one year of OT experience.
• Graduate from an accredited OT program as indicated by transcript or diploma.

Aufgaben

• Coordinate care transitions from home health to outpatient services to meet client-centered goals in living environments.
• Educate clients, families and caregivers on care plans to maximize functional independence.
• Perform outpatient evaluations and develop plans of care for seniors in designated communities or group homes.
• Document treatments and discharge planning using the EMR system.
