Job summary

BAYADA Home Health Care in Sterling, VA is seeking an experienced Occupational Therapist (OT) for a full-time, part-time, or per diem outpatient position serving adult and geriatric residents in senior living communities. • Coordinate care transitions from home health to outpatient services to meet client-centered goals in living environments.
• Educate clients, families and caregivers on care plans to maximize functional independence.
• Perform outpatient evaluations and develop plans of care for seniors in designated communities or group homes.
• Document treatments and discharge planning using the EMR system.

BAYADA Home Health Care is currently seeking an experienced Occupational Therapist, OT, for a Full time, Part Time, or Per Diem opportunity performing Outpatient visits for our Sterling, VA office. This office services adult and geriatric patients on a per visit basis in Senior Living Communities throughout Fairfax, VA.

This unique model combines the personalized care and relationship-building of home-based services with the resources and structure of an outpatient setting. As part of an integrated continuum of care, you'll collaborate closely with our home health therapy team to ensure seamless transitions and ongoing support as residents' needs change. Together, we provide comprehensive, patient-centered care that keeps seniors thriving where they call home.

Occupational Therapist, OT Responsibilities:

• Work closely with your home health therapy partners to appropriately transition care from home health to outpatient services in order to achieve client centered goals in their living environment, allowing the right care, at the right time for each client
• Educate the client, family and community caregivers on the care plans created for your clients in order to achieve a team approach to maximizing functional independence
• Perform outpatient therapy evaluations in order to develop a plan of care for patients in designated senior living communities or in a group home setting for adults with intellectual disabilitiesProfessionally operate at top of licensure utilizing current research-based approaches to care in order to maximize functional independence for your clients
• Perform evaluations, treatments and proper discharge planning for your clients utilizing out outpatient EMR platform to deliver and document on your individualized treatment approaches to meet the needs of your clients

Occupational Therapist, OT Qualifications:

• A current VA Occupational Therapist license.
• Prior home care experience preferred, but not required
• A minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills, organizational skills, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information:

Base Pay, depending upon qualifications:

$48-$51 per hour
