Job summary

Flatwater, Inc., a tribally owned company and a division of Ho-Chunk, Inc., seeks a Full-time Outpatient Nurse Practitioner to deliver direct, non-personal primary healthcare at the Ute Mountain Ute Health Center in Towaoc, Colorado, serving pediatric to geriatric patients.

You will participate in the PCMH team, coordinate care, and document in the Government EHR within 24 hours, ensuring high-quality patient care in a tribal health setting.

Qualifications

• Graduation from an accredited Nurse Practitioner educational program (Master's or Doctoral degree in Nursing).
• Minimum of three (3) years of recent clinical experience as a Nurse Practitioner providing primary care services to pediatric and adult patients.
• Must satisfy all IHS and UMUHC credentialing and clinical privileging requirements prior to commencing direct patient care.

Responsibilities

• Deliver direct outpatient primary care for pediatric, adult, and geriatric patients.
• Order, interpret, and evaluate diagnostic imaging and labs as needed.
• Prescribe medications and interventions in compliance with laws and privileges.
• Collaborate with a PCMH team including physicians, nursing staff, behavioral health, and pharmacists.
• Coordinate referrals and care continuity with external specialists when needed.
• Coordinate emergency stabilization and transfers as required.
• Document encounters and orders in EHR within 24 hours; address chart deficiencies promptly.
• Participate in QA, infection control, safety, and clinical quality programs.
• Comply with IHS, UMUHC bylaws, rules, and safety policies.
