Job summary

Alpha Home Health and Hospice in Oregon seeks a Physical Therapist – Outpatient Neuro to provide skilled PT services to patients with neurological and other diagnoses, promoting functional independence.

Qualifications

• A PT degree from an accredited program is required.
• Oregon Physical Therapist License (and licenses in care delivery states) is required.
• BLS certification is required.

Responsibilities

• Assess and treat patients with diverse diagnoses and disabilities.
• Evaluate patient condition and develop personalized treatment plans.
• Provide professional consultations to maximize patient independence.
