Qualifications

• Bachelor's degree in Occupational Therapy (OT) is required.
• Licensed in the State of Ohio as an OT.
• BLS/CPR certification.
• Valid driver’s license for Home Care assignments and acceptable MVR.

Responsibilities

• Provide direct patient care (90–100%) with evaluation, treatment, and discharge planning.
• Document progress notes and communicate with the care team.
• Mentor OT/OTA students and supervise rehab technicians as needed.
• Support department quality improvement and participate in team meetings.
