Assess, plan, implement, and evaluate patient care using age-specific components
Coordinate care with education, self-management, and patient satisfaction across the continuum

Lead and support teams during clinical routines and emergencies
Participate in care coordination with inpatient and outpatient teams and attend interdisciplinary meetings
