Job summary

MVHC is seeking a Social Worker to join the Behavioral Health Team in Zanesville. This 100% outpatient role offers collaboration with physicians, mid-level providers, licensed social workers, RNs and LPNs, with Epic EMR used for documentation and no call requirements.

Qualified candidates hold an Ohio LSW or LISW license and have at least two years of experience in primary or acute care settings within community mental health.

Qualifications

• Current, unrestricted Ohio license as an LSW or LISW.
• 2 years of experience in a primary care or acute care setting and community mental health center preferred.
• Experience working with all ages, including children.

Responsibilities

• Provide social work services in a 100% outpatient Behavioral Health setting.
• Collaborate with a multidisciplinary team of physicians, mid-level providers, RNs and LPNs.
• Coordinate care and referrals, utilizing Epic EMR for documentation.
• Support patient access to behavioral health resources and follow-up care.
