Job summary

Top Echelon Software LLC is seeking a full-time, in-person Internal Medicine Physician to provide comprehensive outpatient care to adult patients in a mission-driven setting. The role emphasizes prevention, chronic disease management, and community health impact, working with an integrated care team.

Responsibilities include diagnosing and treating conditions, delivering preventive services, documenting encounters in the EHR, and participating in quality improvement initiatives while supporting

Qualifications

• MD or DO from an accredited medical school.
• Board Eligible or Board Certified in Internal Medicine.
• Active state medical license (or ability to obtain).
• Experience in outpatient or community-based care preferred.
• Strong clinical judgment, communication skills, and team-oriented approach.

Responsibilities

• Provide comprehensive outpatient internal medicine care to adult patients.
• Diagnose, treat, and manage acute and chronic medical conditions.
• Deliver preventive care, health screenings, immunizations, and wellness services.
• Develop and manage individualized treatment plans and follow-up care.
• Collaborate with interdisciplinary clinical teams and coordinate specialty referrals.
• Deliver culturally competent care with consideration of social determinants of health.
• Participate in quality improvement initiatives, care coordination, and population health efforts.
• Accurately document patient encounters in the electronic health record (EHR).
• Engage in clinical meetings, care team collaboration, and community health initiatives.
