Job summary

Dignity Health Medical Group in Phoenix, AZ, is seeking a Transitional Care Nurse Practitioner or Physician Assistant for comprehensive primary care in an outpatient clinic. You will manage diagnoses, provide ongoing treatment, and coordinate with a multidisciplinary team.

Qualifications

• Licensed NP or PA in AZ with a Master’s degree from an accredited program.
• Active or eligible license in the state of Arizona.
• National certification in NP/PA area.
• American Heart Association BCLS certification.

Responsibilities

• 100% outpatient clinic responsibilities including assessment, evaluation, diagnostic determination, ongoing treatment, care planning and modification.
• Medications reconciliation, risk assessment and coordination with Acute Care Providers, Transitional Team and Primary Care Physicians.
• Direct contact with patient families and advocates to communicate status and plans.
• Documentation of clinical and professional records to assure care coordination.
• Collaboration with physician/medical director within scope of practice.
• Make referrals to specialists and community agencies as needed.
• Participate in quality assurance and quality improvement programs.
• Develop care plans with multidisciplinary team for patients and families.
