Job summary

Responsibilities

• Participate in a faculty group practice with eleven other internists.

• Increase access to primary care at multiple locations.
• Provide timely transitional follow-up for recently seen patients.
• Help expand transition-of-care services from hospital to outpatient.
• Leverage telemedicine to increase patient access.
• Assist with training and co-management of patients with residents.
