Qualifications

• Master's degree in Social Work from a CSWE-accredited program required.
• Minimum five years hospital-based social work (including discharge planning) or ambulatory clinic experience with five years of supervisory/management experience.
• Adheres to the NASW Code of Ethics and NASW Standards for Social Work Practice in Healthcare Settings.
• Internal applicants are required to attach their three most recent performance appraisals.

Responsibilities

• Fulfills all requirements of Clinical Social Worker in inpatient and ambulatory settings.
• Leads, directs, and manages staff development, performance, and supervision.
• Coordinates and participates in committees, research, and regulatory standards review.
• Supports continuity of patient care by resolving psychosocial issues and removing barriers.
