Qualifications

• Licensed Nurse Practitioner in Delaware or eligible.

Responsibilities

• Conduct patient evaluations and follow-up visits.
• Manage chronic and complex medical conditions.
• Order and interpret laboratory and diagnostic testing.
• Develop and adjust treatment plans in collaboration with supervising physicians.
• Monitor biologic and infusion therapies.
• Provide patient education and counseling.
• Coordinate care with specialists and primary care providers.
• Maintain accurate documentation within the EMR.
