Qualifikationen

• Bachelor’s or Master’s degree in Nutrition, Dietetics, or related field.
• Registered Dietitian Nutritionist (RDN) credential through the CDR.
• Current state licensure or eligibility; BLS certification; ServSafe as required.

Aufgaben

• Conduct nutrition assessments and identify nutrition-related risks.
• Develop, implement, monitor, and adjust individualized nutrition care plans.
• Provide one-on-one and group nutrition counseling for various conditions.
• Educate patients and families on healthy eating and disease management.
• Document assessments and progress in the EHR according to standards.
• Collaborate with physicians and care teams to coordinate patient care.
• Monitor outcomes and evaluate nutrition interventions.
• Address nutrition-related quality measures and social determinants of health.
• Participate in interdisciplinary team meetings and quality improvement.
