Responsibilities

  • Conduct nutrition assessments and identify nutrition-related risks.
  • Develop and adjust individualized nutrition care plans with interdisciplinary teams.
  • Provide one-on-one and group nutrition counseling for patients with acute and chronic conditions.
  • Educate patients and families on healthy eating, disease management, and lifestyle changes.
  • Document assessments and progress in the EHR in line with standards.
  • Collaborate with physicians, nurses, dentists, pharmacists, care coordinators, and others to coordinate care.
  • Monitor outcomes and evaluate nutrition interventions' effectiveness.
  • Support quality improvement initiatives addressing nutrition-related measures.

Position Summary

The Registered Dietitian is responsible for assessing patients’ nutritional needs, developing individualized nutrition care plans, providing nutrition counseling and education, and collaborating with interdisciplinary teams to improve health outcomes. This position promotes disease prevention, chronic disease management, and overall wellness through evidence-based nutrition practices.

Essential Duties & Responsibilities
  • Conduct comprehensive nutrition assessments and identify nutrition-related risks.
  • Develop, implement, monitor, and adjust individualized nutrition care plans in collaboration with interdisciplinary teams.
  • Provide one-on-one and group nutrition counseling for patients with acute and chronic health conditions.
  • Educate patients, families, and caregivers on healthy eating, disease management, and lifestyle modifications.
  • Document assessments, interventions, and progress in the electronic health record (EHR) in accordance with organizational and regulatory standards.
  • Collaborate with Physicians, Care Team Nurses, Dentists, Behavioral Health Providers, Pharmacists, Care Coordinators, Community Health Workers, and other healthcare professionals to coordinate patient care.
  • Monitor patient outcomes and evaluate the effectiveness of nutrition interventions.
  • Support organizational quality improvement initiatives by addressing nutrition-related quality measures, including diabetes, hypertension, obesity, cardiovascular disease, and preventive health screenings.
  • Utilize population health reports to identify patients with care gaps and participate in outreach efforts, as appropriate.
  • Participate in team huddles and interdisciplinary care team meetings.
  • Deliver culturally responsive, trauma-informed, and patient-centered nutrition care that respects patients' beliefs, language, health literacy, and socioeconomic needs.
  • Assess patients for food insecurity and other social determinants of health and collaborate with community resources and internal support services to address identified needs.
  • Conduct in-service training for nursing staff on topics related to nutrition and dietary management.
  • Maintain current knowledge of evidence-based nutrition guidelines and best practices.
  • Comply with all organizational policies, HIPAA regulations, and accreditation standards.
  • Participate in internal quality improvement teams. Works with team members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals for the organization, federal and state laws and regulations, and accreditation standards.
  • Compliance with all state and federal laws and regulations, as they pertain to position including; HIPAA, sexual harassment, scope of practice, OSHA, etc.
  • Other duties as assigned.
QualificationsMinimum Qualifications
  • Bachelor’s or Master’s degree in Nutrition, Dietetics, or related field from an accredited institution.
  • Registered Dietitian Nutritionist (RDN) credential through the Commission on Dietetic Registration (CDR).
  • Current state licensure or eligibility for licensure as required
  • Current Basic Life Support (BLS) certification
  • State licensure or ServSafe certification as required by local laws.
Preferred Qualifications
  • One or more years of clinical nutrition experience in an outpatient, community health, or hospital setting.
  • Experience working with culturally diverse and underserved populations.
  • Bilingual skills preferred.
Knowledge, Skills & Abilities
  • Strong knowledge of medical nutrition therapy and evidence-based nutrition practices.
  • Excellent communication, counseling, and motivational interviewing skills.
  • Ability to build rapport with patients from diverse backgrounds.
  • Strong critical thinking, organizational, and documentation skills.
  • Proficiency with electronic health records and Microsoft Office applications.
  • Ability to work independently while collaborating effectively within a multidisciplinary team.
Physical Requirements
  • Ability to sit, stand, and walk for extended periods.
  • Ability to communicate effectively in person, by telephone, and electronically.
  • May occasionally lift or move up to 25 pounds.
Work Environment

This position is primarily based in an outpatient clinic or healthcare setting and may require travel between clinic locations as needed.
