Job summary

Sky-Lakes-Medical-Center is seeking an Outpatient Dietitian to provide nutrition assessments, counseling, and medical nutrition therapy in primary care clinics. You will develop care plans for diabetes, hypertension, obesity, and cardiovascular conditions, collaborating with a multidisciplinary team to ensure coordinated, patient-centered care.

In this role you will document patient interactions in the EMR, participate in case reviews and health promotion initiatives, and deliver education

Qualifications

• Registered Dietitian with current Oregon licensure.
• Effective communicator with multidisciplinary teams.
• Self-starter able to work independently.
• Preferred: two years’ experience in acute or ambulatory settings.
• Certification: CDR and Oregon licensed Dietitian.

Responsibilities

• Provide individualized nutrition assessments, counseling, and medical nutrition therapy (MNT) to patients in primary care clinics.
• Develop and implement evidence-based nutrition care plans for diabetes, hypertension, obesity, cardiovascular disease, and GI disorders.
• Collaborate with primary care providers and multidisciplinary teams to support coordinated, whole-person care.
• Document all patient interactions in the EMR per clinic policies and standards.
• Participate in case reviews, care team huddles, and health promotion initiatives.
• Deliver nutrition education sessions individually or in groups, in-person and via telehealth.
• Contribute to development of nutrition programs and resources for the clinic population.
• Maintain up-to-date knowledge of current nutrition guidelines and evidence-based practices.

QUALIFICATIONS:

Required: Registered Dietitian (RD) with current Oregon licensure. Must demonstrate a positive attitude, flexibility, and strong problem-solving skills. Self-starter able to work independently and manage patients in diverse outpatient settings. Requires ability to communicate effectively and concisely with a multidisciplinary team to support coordinated, patient-centered care.

Preferred: Two years’ experience in acute or ambulatory setting.

Cert: CDR, Oregon licensed Dietitian

• Document all patient interactions and interventions in the electronic medical record (EMR) in accordance with clinic policies and professional standards.
• Participate in case reviews, care team huddles, and health promotion initiatives.
• Deliver nutrition education sessions individually or in group settings, both in-person and via telehealth.
