Job summary

Beth Israel Lahey Health Primary Care is seeking a Diabetes Educator Team Nurse to educate patients with diabetes, support self-management goals, and collaborate with providers and care teams. This role includes annual wellness visits, chronic care management, and diabetes education across multiple sites.

We value RN or LPN licensure, CDCES certification, strong interpersonal and patient education skills, and proficiency with Microsoft Office applications to document care and deliver education.

Qualifications

• Graduate from an accredited Nursing Program required.
• Bachelor's degree preferred.
• 1-3 years related work experience required.
• Strong interpersonal skills. Strong patient communication and patient education skills.
• Advanced skills with Microsoft applications including Outlook, Word, Excel, PowerPoint or Access.

Responsibilities

• Coordinate and facilitate quality clinical management of a broad based outpatient population across the continuum of care.
• Collaborates with providers to identify needs for preventive care, evidence-based chronic disease management and care coordination.
• Provides general and specific healthcare education.
• Works directly with patients to identify and monitor self-management goals.
• Monitors and ensures follow up of all abnormal test and imaging results under the direction of the provider.
