Job summary

St. Mary’s Medical Center is seeking a full-time Registered Nurse (RN) to join the CVT Outpatient department. The nurse will work collaboratively with a care team to develop patient-centered plans of care and monitor patients’ health using certified health technology.

Requirements include graduation from an approved nursing program, WV RN licensure or compact state license, and BLS. Two years of clinical or case management experience are preferred.

Qualifications

• Graduate of an approved School of Nursing.
• Current licensure as a Registered Nurse (RN) in the state of West Virginia or a valid compact state nurse license.
• BLS required.
• A minimum of two (2) years of experience in a clinical or case management setting preferred.

Responsibilities

• Collaborate as part of a care team to address patients’ needs and create a patient-centered plan of care.
• Monitor the health and well-being of patients using health technology.
• Develop a comprehensive care plan to promote preventive care and support self-management of chronic diseases.
• Collaborate with providers and other caregivers to facilitate needed care.
