Job summary

Prairie Knights is seeking an Out Patient Counselor in Fort Yates, ND. The role provides counseling to chemically dependent patients in an out-patient facility and develops individualized treatment plans to support a drug-free lifestyle.

Core duties include substance use disorder screenings, biopsychosocial assessments, DSM-5 and ASAM-informed recommendations, coordinating admissions, and arranging referrals to inpatient or outpatient services.

Qualifications

• Bachelor's Degree required.

Responsibilities

• Conduct substance use disorder screenings, assessments, and evaluations per program policies.
• Complete biopsychosocial assessments, substance use histories, and intake interviews.
• Utilize DSM-5 and ASAM Criteria to determine levels of care and treatment recommendations.
• Coordinate admission process for eligible clients.
• Develop preliminary treatment recommendations based on assessment findings.
• Coordinate referrals to inpatient, outpatient, aftercare, mental health, housing, and other services.
• Provide crisis intervention and immediate support for clients.
• Maintain accurate, complete, and timely client records.
• Participate in multidisciplinary team meetings, staffing, and treatment planning.
• Protect client confidentiality and privacy at all times.

SCOPE OF WORK:

The Out Patient Counselor provides counseling to patients in an out-patient facility, which are chemically dependent and develops a treatment program on individual’s needs. Will assist patients in developing an understanding of chemical dependency and facilitates effective methods to maintain a Drug-Free lifestyle.

KEY DUTIES & RESPONSIBILITIES:

• Conduct substance use disorder screenings, assessments, and evaluations in accordance with program policies and procedures.
• Complete comprehensive biopsychosocial assessments, substance use histories, and intake interviews.
• Utilize DSM-5 and ASAM Criteria to assist in determining appropriate levels of care and treatment recommendations.
• Coordinate and facilitate the admission process for eligible clients.
• Develop preliminary treatment recommendations based on assessment findings.
• Coordinate referrals to inpatient, outpatient, aftercare, mental health, medical, housing, employment, educational, legal, social service, and community resources.
• Provide crisis intervention and immediate support for clients presenting with urgent behavioral health or substance use concerns.
• Maintain accurate, complete, and timely client records in accordance with program requirements and professional standards.
• Participate in multidisciplinary team meetings, staffing, case reviews, and treatment planning discussions.
• Maintain accurate, complete, and timely client records in accordance with program requirements and professional standards.
• Protect client confidentiality and privacy at all times.

PREFERRED QUALIFICATIONS:

• Bachelor's Degree
• Valid Driver’s License

SHIFT: Monday through Friday, 8:00 a.m. to 4:30 p.m.
