Job Title: Outpatient Coordinator (Hybrid Position)

Location: West Covina, CA

Schedule: Monday–Friday, 8:30 AM – 5:00 PM (30-minute lunch)

Work Arrangement: Hybrid (In-office Tuesday–Thursday; remote Monday & Friday)

Equipment Provided

Pay: $23/hour

Contract: 90-day temporary assignment with potential for extension

Start Date: 7/1/2025

Position Summary

We are seeking a detail-oriented and compassionate Outpatient Coordinator to support our wellness outreach and care management efforts. This role is ideal for someone with a healthcare experience who enjoys working with elderly populations and thrives in a fast-paced, collaborative environment.

Key Responsibilities

  • Conduct wellness outreach calls to members and request medical records from providers (5–10 per day).
  • Schedule transportation for members and coordinate with hospitals to obtain records.
  • Verify member eligibility and coordinate with insurance providers.
  • Prepare member charts and documentation for case managers.
  • Use templates and review prior notes during wellness checks.
  • Maintain accurate records and follow up on member interactions.
  • Support case managers with scheduling services such as home health, DME, and specialist appointments 1.


Required Qualifications

  • Experience in a medical office or healthcare setting, especially working with providers and members.
  • Typing speed of at least 35 WPM with accuracy.
  • Proficiency in Microsoft Excel and Word (including report creation and spreadsheet management).
  • Strong communication and organizational skills.
  • Bilingual in Spanish is a plus.
  • Familiarity with HMO insurance plans is highly preferred.
  • Knowledge of Access Express is a bonus


Training Program

  • Week 1: System training (in-person and WebEx).
  • Weeks 2–3: Departmental training with team lead.
  • Weeks 4–6: Mentorship-based training.


Team Environment

You’ll join a collaborative team of 7–8 coordinators and an equal number of case managers. We value empathy, effective communication, and a shared commitment to member care.

Performance Expectations

  • Minimum of 15 member interactions per day (eligibility checks, outreach, PCP coordination, etc.).
  • Maintain compliance with turnaround times and documentation standards.
  • Demonstrate professionalism, integrity, and respect in all interactions.


Pay And Benefits

The pay range for this position is $23.00 - $23.00/hr.
