Job summary

The Winslow Indian Health Care Center seeks a Clinical Pharmacist to provide direct outpatient pharmaceutical care under the supervision of the Chief Pharmacist. The role focuses on managing selected chronic disease patients, optimizing therapy, and supporting acute care patients while upholding WIHCC standards.

The pharmacist will document activities, monitor treatments, counsel patients, and collaborate with physicians to develop protocols and improve care quality within the service unit.

Qualifications

• Doctor of Pharmacy degree required; licensed by State Board of Pharmacy.

Responsibilities

• Provide direct pharmaceutical care in outpatient clinic.
• Monitor and optimize drug therapy for chronic disease patients.
• Counsel patients on medications and adverse effects.
• Document clinical activities in medical records.
• Collaborate with physicians to develop protocols and improve care quality.
