Job summary

The Epiphany Center is seeking an Outpatient Case Manager in San Francisco, CA. You will join the Family Residence Outpatient Program and provide comprehensive case management and recovery support to clients and families.

You will assess needs, connect clients with resources, and coordinate with community providers while maintaining confidential records in a trauma-informed team environment. This is a full-time, on-site role with standard daytime hours.

Qualifications

• High school diploma or equivalent is required.
• 2+ years of relevant experience in behavioral health, substance use recovery, case management, social services, or related setting.

Responsibilities

We're looking for an Outpatient Case Manager to join our Family Residence Outpatient Program. In this role, you'll provide comprehensive case management and ongoing recovery support, helping clients connect with resources, navigate services, develop after-care plans, and work toward their individual goals.

What You'll Do

As an Outpatient Case Manager, you'll work with the Family Residence Outpatient Program team to provide comprehensive case management and recovery support to clients and families.

• Maintain appropriate professional boundaries and client confidentiality.
• Participate in required staff meetings, trainings, and program activities.
• Follow Epiphany Center policies, safety procedures, confidentiality requirements, and trauma-informed practices.

Compensation & Schedule

Pay Range: $30.00-$33.00 per hour

This is a full-time, on-site position.

Schedule: Monday-Friday, 8:30 a.m.-5:00 p.m.
Location: San Francisco, CA - On-Site
Program: Family Residence Outpatient Program

Requirements

What You'll Bring

• High school diploma or equivalent.
• At least 2 years of relevant experience working in behavioral health, substance use recovery, case management, social services, or a related setting.
• Current registration or certification as an alcohol and other drug (AOD) counselor through CAADE, CCAPP, or CADTP.
• Strong communication, organization, documentation, and interpersonal skills.
• Ability to work effectively as part of a multidisciplinary, trauma-informed team.
• Ability to establish and maintain professional boundaries while building respectful relationships with clients from diverse backgrounds.
• Basic computer proficiency and ability to maintain timely and accurate documentation.
• CPR and First Aid certification as required for the position.
• Ability to meet all applicable pre-employment and program requirements.
