We are currently seeking an Outpatient Case Manager for Riveredge Hospital. Simply apply to the posting and discuss your interests with the hiring manager if you are selected to come in for an interview.

TheOutpatient Case Manager provides administrative and direct client tasks that positively supports hospital operations and is consistent with the philosophy and objectives of Riveredge Hospital.

Essential Functions and Responsibilities:

  • Complete Medication Prior Authorizations
  • Complete reminder calls for patients with injections and lab needs
  • Organize/scan labs received and organize LAIs delivered to the clinic
  • Completing welcome calls for New Evals and offer additional resources (groups/individual therapy)
  • Take calls transferred to intake for clients requesting new services, further streamlining the process and removing barriers to scheduling
  • Call “at-risk” appointments and assist with removing barriers to attending appointments
  • Register patients for groups
  • Organize LAIs delivered to clinic
  • Complete weights and vitals
  • Complete verification letters/forms for patients
  • Complete DCFS consents and monitor guardianship paperwork
  • Assist with crisis management and step ups
  • Review Social Determinants of Health and Elder Abuse Screenings and provide resources as needed.
  • Open and close clinic on assigned days including providing necessary documents to ensure clinic runs efficiently and effectively (e.g., sign-in sheets, billing log, necessary assessments, provider schedules, necessary charts) prior to start of shift.
  • Answer telephone calls, route calls, provide information, take messages, and check voicemails and emails in accordance with outpatient procedures.
  • Assist in scheduling and adjusting consumer appointments to maximize consumer care.
  • Greet visitors and consumers entering the outpatient and alert appropriate staff of consumer/visitor arrival.
  • Register consumers in appropriate systems, provide necessary paperwork and ensure all consumers are in proper system for billing.
  • Collect and maintain current insurance information in consumers' files.
  • Process consumers payments or contact business office.
  • Scan billing and other essential documents to necessary parties after close of clinic
  • Use computer, fax, or courier service to transmit information or documents.
  • Adhere to procedures and policies necessary to maintain HIPAA compliance.
  • Other duties as assigned.

Qualifications

Education/Job Experience/Certification:

  • Bachelor's degree in social work, psychology, or related
  • At least 1year experience in case management
  • At least 1 year experience in a psychiatric setting is preferred
  • Spanish speaking preferred.

  • Excellent telephone etiquette
  • Ability to interact effectively with persons in crisis or resistant toward organization.
  • Excellent organizational and time management skills
  • Professional office administration.
  • Strong interpersonal and communication skills required.
  • Is open and respectful of cultural and socioeconomic characteristics of all and is willing and competent to work with a diverse client population.
  • Effective verbal and written communication skills.
  • Ability to work both independently and a part of a high functioning team.
  • Experience and ability to use computers for data entry, communication and report writing.
  • Knowledge of all code procedures with the ability to react calmly and effectively in emergency situations.
