Job summary

Concord Hospital is seeking a licensed mental health professional to provide clinical assessment and outpatient treatment as part of an interdisciplinary team.

The role involves developing patient-centered treatment plans, delivering direct care, and contributing to education of Behavioral Health Interns and Family Medicine Residents. Licensure eligibility in New Hampshire is required within two years of hire.

Qualifications

• Five to six years of formal training or education beyond the high school level (e.g., Masters Degree or five-year program in a mental health discipline).
• Licensure required to obtain: Must be license eligible. Licensed by the state of New Hampshire to perform mental health practice within two years of hire date.
• At least two years practicing mental health counseling preferred.

Responsibilities

• Conducts bio-psycho-social mental health assessments of patients and develops patient-centered treatment plans.
• Administers direct patient care.
• Participates in education and training of Behavioral Health Interns and Family Medicine Residents.
• Documents care provided according to Medicare and other compliance standards, and CHMG and practice standards.
• Monitors schedule in order to maximize patient access.
