Qualifications

• Masters degree or higher in a mental health discipline.
• License eligibility in New Hampshire within two years of hire.
• At least two years of mental health counseling experience preferred.
• Ability to work as part of an interdisciplinary treatment team.
• Compliance with Medicare and CHMG practice standards.

Responsibilities

• Conducts bio-psycho-social mental health assessments of patients and develops patient-centered treatment plans.
• Administers direct patient care.
• Participates in education and training of Behavioral Health Interns and Family Medicine Residents.
• Documents care provided according to Medicare and other compliance standards, and CHMG and practice standards.
• Monitors schedule in order to maximize patient access.
