Mercyhealth Wisconsin and Illinois is seeking a Behavioral Health Case Manager BS - Outpatient to assess and assist with mental health needs. The role acts as a patient and family case worker, coordinates program activities, and advocates within an interdisciplinary team to improve outcomes.

Qualifications

• Bachelor’s Degree in mental health or behavioral health field.
• Pursuing advanced degree or licensure is preferred.
• TACT Level 2 certification within 90 days; CPR/BLS preferred.

Responsibilities

• Collaborate with physicians and health care staff to understand social and emotional factors affecting patients.
• Assess ongoing treatment needs, prepare histories, discharges, and care plans.
• Develop dispositional plans and coordinate care with departments.
• Maintain strong interdepartmental communication and liaise with psychiatrists.
• Support treatment planning and discharge/aftercare coordination.
