Responsibilities

Hampton Behavioral Health Center is a private behavioral healthcare center offering advanced behavioral health and diagnostic treatment services for adults, adolescents, and older adults in both inpatient and outpatient programs. Our mission is centered on excellence in diagnosis, treatment, and education.

We are seeking a dynamic and talented Assessment Coordinator/Patient Navigator for Hampton Counseling Center to assist with patient intakes in our Outpatient department.

Shift:

  • Full Time
  • 8:00 am – 4:30 pm, Monday - Friday
Responsibilities
  • Facilitate the efficient operation of the Hampton Counseling Center and Admissions Department by providing excellent customer service to referral agencies and internal departments.
  • Complete therapeutic face-to-face level of care and biopsychosocial assessments for patients seeking psychiatric evaluation, including those with co-occurring substance abuse.
  • Provide treatment recommendations based on assessment outcomes and initiate preliminary treatment plans.
  • Offer crisis and supportive therapy to patients and families.
  • Ensure accurate and timely documentation.
  • Evaluate referrals for outpatient services and coordinate with referral agencies for admissions and transfers.
  • Provide clinical assessment information and initial treatment recommendations to insurance companies for authorization.
  • Handle inquiry calls and referral information with excellent communication skills.
  • Provide brief therapeutic and crisis interventions during inquiry calls.
  • Complete clerical processes for outpatient admissions using MedSeries.
  • Assume clinical responsibility for biopsychosocial assessments and develop initial treatment plans.
  • Participate in discharge planning and multidisciplinary treatment teams.
  • Deliver evidence-based therapeutic interventions, including group and family therapy.
  • Discuss PHP/LOP services with inpatients and facilitate proper referrals.
  • Assist inpatient treatment teams with step-down processes, case management, and discharge planning.
  • Enter data, evaluate programs, and manage outcomes via Midas.
  • Assess patient needs based on age-specific growth and development principles.
  • Report to the Directors of Hampton Counseling Center and collaborate with the Admissions and Access Center to ensure compliance.
  • Maintain flexibility to cover multiple outpatient locations.
  • Maintain strict confidentiality.
Knowledge and Skills
  • Knowledge of crisis intervention techniques.
  • Strong verbal and written communication skills.
  • Proficiency in computer skills, data entry, word processing, and spreadsheets.
  • Ability to multitask and organize effectively.
