Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant to provide in-house occupational therapy services at a Skilled Nursing Facility in Newark, OH. This PRN position supports resident care and therapy plans in collaboration with supervising therapists.

The OTA must hold an active license and participate in ongoing continuing education to maintain licensure and contribute to high-quality resident care.

Qualifications

• Associate's degree in Occupational Therapy is required.
• Active OTA licensure in state(s) of practice is required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screenings of residents to determine best intervention per scope of practice.
• Perform treatment plans and follow short- and long-term goals per plan of care.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and healthcare team.
• Establish individual resident therapy schedules in collaboration with team members.
• Document and bill occupational therapy services as per plan of care.
